Vaccine Waiver/Exemption
The state of Texas allows for parents to obtain a waiver exempting their students from the vaccine requirements. The link to the waiver information is provided. You must complete and submit the application. The state will mail you a certificate that must be notarized. Submit the notarized waiver to your campus nurse. Waivers are good for 2 years and then parents must reapply.
