This clinic is for children and adults wanting to get the flu vaccine.

Please bring a copy of your insurance card, driver’s license and all paperwork completed.
Mask will be required for everyone 2 years of age and older, if you do not have one, they will provide one. Parents will need to accompany the child. The link below will direct you to the required paperwork.
